Obtaing National Identity Card for the first time
Basic requirements to be met to apply for a National Identity Card
Every person who is a citizen of Sri Lanka and who has attained or attains the age of 15 years shall apply for a National Identity card
Documents to be submited
- Duly completed application (Form D.R.P/1,7,8) (This form can be obtained from the Identity Card Section of the Divisional Secretariat or from the Grama Niladhari.)
- ICAO Standard photograph obtained within 06 month period.
- Birth certificate or extract of the birth registry certified by the Additional District Registrar
If you are under 50 and do not have a birth certificate, a certificate of presumed age.
If you are over 50 years of age and do not have a birth certificate, please submit one of the following documents along with the blank result sheet:
School Leaving Certificate, Baptismal Certificate, Birth Certificate of a child stating the applicant's date of birth, Extract from the Birth Certificate, Estate Leaving Certificate, Certified copies of relevant pages of the passport, Registration Card issued by the Estate Superintendent, Birth Certificate
- Citizenship certificate if born abroad
- If you are a Buddhist monk, a novice certificate or a certificate of ordination
- The fee for obtaining the National Identity Card for the first time and the registration stamp fee of Rs. 340/- should be paid to the Grama Niladhari
- If the person has completed 16 years of age, the appropriate prescribed declaration (available from the Grama Niladhari) along with the receipt of payment of the fine of Rs. 2500/- (if the person is a low-income earner, the Grama Niladhari certified concessional fine declaration along with the receipt of payment of the fine of Rs. 250/-)
It is possible to carry out the necessary work by submitting these documents to the Grama Niladhari of the division
National Identity Card Amendments
Documents to be submited
- Duly completed application (Form D.R.P/1,7,8) (This form can be obtained from the Identity Card Section of the Divisional Secretariat or from the Grama Niladhari.)
- ICAO Standard photograph obtained within 06 month period.
- Birth certificate or extract of the birth registry certified by the Additional District Registrar
If you are under 50 and do not have a birth certificate, a certificate of presumed age.
If you are over 50 years of age and do not have a birth certificate, please submit one of the following documents along with the blank result sheet:
School Leaving Certificate, Baptismal Certificate, Birth Certificate of a child stating the applicant's date of birth, Extract from the Birth Certificate, Estate Leaving Certificate, Certified copies of relevant pages of the passport, Registration Card issued by the Estate Superintendent, Birth Certificate
- If you are a Buddhist monk, a novice certificate or a certificate of ordination
- If the Buddhist monk has been ordained, the ordination certificate issued by the Department of Buddhist Affairs.
- Old National Identity Card
- If the National Identity Card number is unclear, a document stating the National Identity Card number (copy of a bank book, copy of a drivings license, copy of a foreign passport)
- Original marriage certificate if husband's name must be included
- The amount of Rs. 640/- for the fee for the revision of the National Identity Card and the registration stamp fee.
It is possible to carry out the necessary work by submitting these documents to the Grama Niladhari of the division
National Identity Card Lost
Documents to be submited
- Duly completed application (Form D.R.P/1,7,8) (This form can be obtained from the Identity Card Section of the Divisional Secretariat or from the Grama Niladhari.)
- ICAO Standard photograph obtained within 06 month period.
- Birth certificate or extract of the birth registry certified by the Additional District Registrar
If you are under 50 and do not have a birth certificate, a certificate of presumed age.
If you are over 50 years of age and do not have a birth certificate, please submit one of the following documents along with the blank result sheet:
School Leaving Certificate, Baptismal Certificate, Birth Certificate of a child stating the applicant's date of birth, Extract from the Birth Certificate, Estate Leaving Certificate, Certified copies of relevant pages of the passport, Registration Card issued by the Estate Superintendent, Birth Certificate
- If you are a Buddhist monk, a novice certificate or a certificate of ordination
- If the Buddhist monk has been ordained, the ordination certificate issued by the Department of Buddhist Affairs.
- Copy of the police complaint
- A document stating the identity number (copy of a bank book, copy of a driver's license, copy of a passport)
- Original marriage certificate if husband's name must be included
- The fee for the revision of the National Identity Card and the registration stamp fee of Rs. 1140/- should be paid to the Grama Niladhari.
It is possible to carry out the necessary work by submitting these documents to the Grama Niladhari of the division
Incorporation / alteration of the name of a birth certificate
For those over 7 years -
- The Christian Certificate-Student Tester
- Newspaper advertisement - Bank Book
- National / Posture Identification Card - Interim Certificate
- Report from the government or other institution
- Certified copy of voting register
You must submit at least three of the above documents
other than that
- Application
- Original birth certificate to be amended
- Grama Niladhari letter with relevant Divisional Secretary for signature that both persons are identified as identical.
NB-:
- If the name under the age of 21 is to change the name, the statement should be forwarded by the parent / guardian of their respective children.
- It is recommended that the new name be used for at least one year.
Children below the age of 7 years
- Original birth certificate to be amended
- Parent's Open Certificate
- Mother's / father's National Identity Card
- Affidavit that there is no objection to father and mother's protest.
This declaration should be handed over by the child's parent / guardian.
Correction of a mistake that occurred in the mother's area of birth certificate
- Original birth certificate to be corrected
- Original mother's birth certificate
- Copy of mother's national identity card
- Original parents' public certificate
To Include / rectify the father's area, Grand farther's area and civil status in a birth certificate
- The original copy of the birth certificate which can be corrected
- Original father's birth certificate
- Original copy of the parents' certificate
- Mother / father's National Identity Card copy
Registration of past births
- Birth Description (Original copy)
- Photo vaccine card copy
- If the parent is married, the marriage certificate of the marriage certificate
- Original Birth Certificates of Parents
- Photocopies of Mother and Father National Identity Cards
- If the birth is less than 1 year, stamps of Rs. 50
- 100 stamps if more than 1 year
Please bring the above documents to the District Registrar of the Department with Parent and child.
Registration of past deaths
- Original death certificate of the hospital if died in a hospital
- Certificate of Grama Niladhari
- Condemnation / death ads
- Cash receipt
- Letter from the Chief Incumbent of the temple
- If the death is less than 1 year, if the stamp is more than Rs. 50 / sterling over 1 stamp
- Birth certificate of the person / marriage certificate and a copy of the National Identity Card
